P0141 code. what else should I check?

I'm getting a DTC P0141 code (O2 Sensor Heater Circuit Malfunction Bank 1 Sensor 2) on my Highlander '21.
I had this sensor replaced around late 2024, and it's popping up again! I can clear the check engine light, and it'll be fine until I hit stop and go traffic or speeds below 50 km/h, then it's just back on. Same code, P0141.

any ideas on where to start looking beyond just the sensor itself? Thanks for any suggestions!
 
If that code keeps popping up even after changing the sensor, the problem might be the wiring or the plug, like a loose connection or damaged wires. Try ti double check the fuse and make sure the sensor you got is the right one
 
Why would the code only come back in stop-and-go or low-speed driving, maybe heat soak or voltage drop is messing with the circuit at those times?
 
I had the same issue on my '19 RAV4, I replaced the sensor twice, but it was actually damaged wiring near the rear subframe causing intermittent shorts. I fixed the harness and the problem never came back.
 
I faced a similar issue with my 2020 SUV. I tried swapping out sensors, but nothing seemed to work. Eventually, I discovered that a frayed wire tucked away near the frame was causing shorts whenever the car slowed down. So, I'd suggest taking a close look at the wiring harness and connectors before you decide to buy a new sensor
